dc.description.abstractThis study is premised on the principle that all accidents are preventable and reviews several of the existing causation theories and models. These were compared against the theories and models reviewed, in order to determine if the location of root causes identified during actual investigations are upstream or downstream to the accident event. It is important to note where the root causes are being identified in the process as it will determine whether the identified corrective action is aligned in the prevention or the reduction of the accident.en_US
